GPs given access to real-time information on patients in hospital

GPs in Derbyshire are to be provided with a real-time view of information on any of their surgery’s patients that are in hospital. At a glance, surgeries will able to see which patients under their practice have been admitted to hospital, what there are in for and when they are likely to be discharged.

Derby Teaching Hospitals NHS Foundation Trust already benefit from Hospedia’s real-time In-Patient Flow Manager solution, which can automatically produce e-discharge summaries as well as deliver discharge text alerts to community care.

Taking this solution further, and supporting continuity and integration of the patient care pathway, GPs in Derbyshire are to be provided with a real-time view of information on any of their surgery’s patients that are in hospital. At a glance, GPs will be able to see which patients under their practice have been admitted to hospital, what they are in for and when they are due for discharge.

The solution is an extension of the already successful Patient Flow Manager solution from Hospedia, and also provides live functionality for the hospital clinician treating the patient, and the GP, to communicate over the patient’s care. It also means that social care can be instantly alerted, should one of their patients be admitted, saving valuable time and preventing unnecessary home visits.

The initial pilot started on Monday 24th October across just 3 surgeries in Derbyshire, and within the first 24hrs of the pilot, at just one of the surgeries, the new functionality prevented 3 unnecessary patient home visits – as the patients had already been admitted to hospital, which was flagged immediately in real-time to the surgery. Initial results of the pilot are very exciting and a roll out to all 57 surgeries within the Trust is planned in December.
